> The Medicines and Healthcare products Regulatory Agency (MHRA) serves as the primary regulator for medical devices within the United Kingdom. This guidance outlines the medical device vigilance system, a framework designed to ensure safety through the reporting and evaluation of adverse incidents and Field Safety Corrective Actions (FSCAs). While this document serves as a general regulatory overview rather than an inspection report for a single entity, it establishes clear mandates for all manufacturers operating in the UK market. Under the established regulatory framework, manufacturers are required to implement robust post-market surveillance systems. Specifically, they must notify the MHRA of any incidents involving their devices that meet defined reporting criteria. This includes any adverse events or corrective actions taken to mitigate risks in the field. The guidance emphasizes that the evaluation of these incidents is critical for maintaining public safety and device efficacy. Required actions for manufacturers include the continuous monitoring of device performance and the timely submission of reports to the MHRA. Failure to comply with these post-market surveillance requirements constitutes a regulatory breach. By adhering to the vigilance system protocols, companies ensure that the MHRA can effectively monitor the market and respond to potential safety issues. This document provides illustrative examples of reportable incidents to assist manufacturers in maintaining compliance with UK safety standards.

## Content

The Medicines and Healthcare products Regulatory Agency (MHRA) is the regulatory authority for the UK medical device market. Once a medical device has been placed on the UK market, the manufacturer must submit reports to the MHRA when incidents that involve their device occur in the UK and meet the criteria for reporting (see Medical devices: post-market surveillance requirements ). The notification and evaluation of adverse incidents and FSCAs involving medical devices is known as the medical device vigilance system. The guidance shows examples of incidents that manufacturers must report to the MHRA under this system.
