FDA 483 - E. Lavern Martin Farm - April 25, 2011

On April 25, 2011, an FDA inspection of E. Lavern Martin Farm, a shell egg producer in Lebanon, PA, identified seven observations. Elmer Lavern Martin, Owner/SE Prevention Plan Administrator, was issued the Form 483.

Observation 1 noted that environmental testing for Salmonella Enteritidis (SE) was performed on February 14, 2011, before the laying hens reached the required 40-45 weeks of age. Observation 2 indicated a failure to implement appropriate fly monitoring methods from July 9, 2010, to January 30, 2011. Observation 3 cited two potential pest harborage areas with general debris and farm equipment: at the rear of the poultry house along the north wall and on the east side near the feeding mill and egg packing house.

Observation 4 stated that supervisory personnel did not review refrigeration, rodent, and fly monitoring records from July 9, 2010, to April 25, 2011. Observation 5 found that refrigeration logs from July 9, 2010, to April 1, 2011, lacked the signature or initials of the person performing the operation.
