During an inspection conducted from March 22-24, 2011, at Smith's Quality Eggs, LLC, a shell egg grower located at 7047 Kelly Rd, LaFayette, NY 13084, the FDA observed five violations.

Observation 1 noted a lack of records documenting compliance with biosecurity measures, specifically regarding employee use of boot dipping stations, despite monitoring of station changes. Observation 2 indicated inadequate practices to prevent cross-contamination when people move between poultry houses. The firm's biosecurity procedure for boot dipping stations lacked monitoring for disinfectant effectiveness and visible organic matter.

Observation 3 found that records documenting compliance with refrigeration requirements were not maintained, specifically for the delivery truck's actual ambient cooling temperature versus the unit's setting. Observation 4 stated that the written SE plan lacked the signature(s) of the person(s) administering it, only carrying the typed name of the firm owner, Roy E. Smith. Finally, Observation 5 identified that required records, including those for stray animal sightings, rodent/fly monitoring, and footbath changes, did not include the time of each activity.
