Olympus Medical Systems Corporation – FDA Warning Letter, Dec 16, 2025
Between October and November 2025, the FDA inspected six Olympus Medical Systems Corporation facilities in Japan, Germany, and the U.S. The resulting Warning Letter identifies systemic failures to comply with Quality System regulations under the Federal Food, Drug, and Cosmetic Act. Major violations include inadequate design validation for devices like insufflators and bronchoscopes, which led to serious risks including patient deaths and multiple Class I recalls. Olympus also failed to properly control nonconforming products, such as distributing improperly reworked surgical needles and miswired generators. Furthermore, the company did not adequately validate critical production processes, such as sterile barrier heat sealing and component cleaning. The FDA highlighted significant weaknesses in the company"s Corrective and Preventive Action (CAPA) programs, noting that Olympus frequently failed to identify root causes, verify the effectiveness of corrections, or properly analyze complaint trends. While Olympus has proposed global remediation efforts, the FDA found current responses insufficient due to a lack of objective evidence. The company must now demonstrate comprehensive corrections to its manufacturing and quality oversight processes to ensure patient safety and regulatory compliance.
